As a Nurse Case Manager Specialist at Bristol Hospice, you will have the opportunity to make a meaningful impact in the lives of our patients and their families. This role requires a compassionate and dedicated individual who is passionate about delivering exceptional patient-centered care.

Job Responsibilities
  • Create and initiate care plans through assessing the history of current and previous illnesses
  • Spend quality one-on-one time with patients and their families to support everyday needs
  • Care for patients by providing initial and ongoing assessments of patient and family needs
  • Record important health information including temperature, pulse, respiration, and blood pressure
  • Administer medications and treatments as prescribed by the physician
  • Provide health care instructions to the patient as appropriate
  • Record symptom management changes and outcomes
  • Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given
Qualifications
  • An enthusiasm for providing the best possible care to patients and their families
  • An RN license in good standing
  • 1 year of experience in hospice or home health preferred
  • A valid driver's license
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills

Compensation

The estimated salary for this position is around $85,000 per year, depending on experience and location. Bristol Hospice offers a competitive compensation package that includes benefits such as tuition reimbursement, PTO, and paid holidays.
