Competitive Salary and Benefits

The estimated salary for this position is around $65,000 - $85,000 per year, depending on experience and location. We offer competitive benefits that support and promote healthy lifestyle choices, including:

Job Description

The Registered Nurse (RN) Case Manager will be the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode. This role requires strong communication and organizational skills, as well as the ability to consult with physicians and office staff as needed.

Responsibilities

The key responsibilities of this role include:

  • Performing visits and completing coordination of client care
  • Overseeing the frequency of visits for the episode
  • Consulting with physicians and office staff as needed
  • Accurately collecting OASIS data
  • Ensuring medication profiles remain current
  • Reporting lab values to physicians in a timely manner
  • Attending weekly case conference and monthly case manager meetings

Qualifications

To be eligible for this role, candidates must:

  • Be currently licensed as a Registered Nurse (RN) in the state of employment
  • Have a minimum of one year of clinical experience preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Possess a valid state driver's license and automobile liability insurance
