Job Description:

We are seeking a highly skilled and dedicated Medical Case Manager to join our team at St. David's South Austin Medical Center.

About the Job:

The Medical Case Manager will play a crucial role in ensuring high-quality, patient-centered care by managing rehabilitation cases to achieve optimum outcomes. This position requires strong communication and organizational skills, as well as the ability to work collaboratively with patients, families, caregivers, and healthcare professionals.

Responsibilities:
  • Provide program orientation to patients and their families, explaining the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
  • Document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences.
Requirements:
  • 2+ years' experience in case management in a hospital setting required.
  • InterQual experience preferred.
  • Associate Degree in Nursing or Nursing Diploma required.
  • Bachelor's Degree in Nursing preferred.
  • Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
  • Certification in Case Management preferred.
