The Compassus Hospice Social Worker is a vital member of our interdisciplinary team, responsible for providing exceptional patient support and promoting the highest level of care. As a skilled social worker, you will work closely with patients and their families to address their psychosocial needs, ensuring they receive the best possible support throughout their journey.

Key Responsibilities
* Assess and Address Psychosocial Needs: Conduct thorough assessments to identify patients' psychosocial needs and develop tailored plans to address these concerns.
* Counseling and Support Services: Provide counseling and support services to patients and their families, helping them cope with the challenges of illness and end-of-life care.
* Community Resource Coordination: Facilitate connections to community resources, ensuring patients have access to necessary services and support.
* Collaboration and Communication: Work effectively with the interdisciplinary team to coordinate care and communicate patient needs to ensure seamless support.
Requirements
* Education and Experience: A minimum of a Bachelor's degree in Social Work, Psychology, Sociology, or a related field, with at least one year of experience in healthcare settings. Licensure may be required by state rules. Supervisory oversight by an MSW for hospice patient care planning and counseling needs is expected.
* Skills and Qualifications: Excellent communication and interpersonal skills, with the ability to empathize with patients and their families. Strong organizational and time management skills, with the capacity to prioritize tasks and manage multiple responsibilities.
Salary and Benefits
We offer a competitive salary range of $65,000 - $80,000 per annum, depending on experience. Work Environment
This role requires regular standing, walking, and manual dexterity. Adaptations can be made to accommodate team members with disabilities. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
