Medical Social Worker Job Description

We are seeking a skilled Medical Social Worker to join our team at Idaho Home Health and Hospice. This role requires a compassionate individual who can provide medical social services to patients in need.

Job Summary

The ideal candidate will have a Bachelor's Degree in social work or a related field, with one year of experience in a healthcare setting. The successful applicant will be responsible for assessing the social and emotional needs of patients, developing plans to address these needs, and providing supportive services as needed.

Key Responsibilities
  • Conduct thorough assessments of patients' social and emotional needs
  • Develop and implement plans to address these needs, in collaboration with other healthcare professionals
  • Provide supportive services, including counseling and education, to patients and their families
  • Work closely with the Interdisciplinary Group to develop comprehensive care plans
  • Establish relationships with community resources to support patient needs
Requirements
  • Bachelor's Degree in social work or a related field
  • One year of experience in a healthcare setting
  • CPR certification
  • Valid driver's license and reliable transportation
  • Licensure as a Social Worker (where required)
