Compassus Hospice Care Coordinator

**Job Summary:**
We are seeking a skilled Hospice Social Worker to join our team at Compassus. As a key member of our interdisciplinary team, you will provide compassionate care and support to patients and their families in need of end-of-life services.
Responsibilities:
- Model the Compassus values of compassion, integrity, excellence, teamwork, and innovation while promoting our philosophy using the 6 Pillars of Success as the foundation.
- Provide preventive, educational, and evaluative treatment services to meet the psychosocial needs of patients and their families.
- Explain hospice services and Medicare benefits to patients and families, obtain informed consent and election of benefits documents, and submit relevant documentation and paperwork timely according to company policy.
- Participate as a member of the IDT, including development and implementation of the plan of care, and effectively communicate patient and family needs to the IDT.
- Collaborate with the IDT to coordinate psycho-social care and support for patients and families to ensure appropriateness, continuity, and quality of care.
- Maintain updated, comprehensive care plans that reflect current problems, goals, and interventions for patients.
- Complete initial and ongoing psycho-social assessments for patients, identify concerns/issues related to financial means, insurance, living arrangements/placement for long-term care, caregiver stressors, pre-bereavement and anticipatory grief, and community resource needs.
- Facilitate placement in long-term care as warranted and monitor adjustment.
- Support and facilitate advance planning, including living will/POA documents, funeral arrangements, memorial services, and body/organ donation.
Requirements:
- Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or other field related to social work with one (1) year of social work experience in a healthcare setting required; licensure if required by state rules.
- Master's degree in Social Work with one (1) year of social work experience in a healthcare setting highly preferred; and licensure if required by state rules.
