Family Nurse Practitioner or Physician Assistant

We are seeking a dedicated and compassionate Family Nurse Practitioner or Physician Assistant to join our team at LifeLong Medical Care. As a primary care provider, you will be responsible for delivering high-quality, patient-centered care to our underserved communities in West Berkeley.
Responsibilities- Provide preventive and primary health services in a Community Health Clinic setting.
- Act as an advocate for clients and consult with other health team members.
- Maintain client records and perform history and physical examinations.
- Order and/or perform laboratory and screening tests, and recommend treatment and medication.
- Provide education, verbal and written, as well as counseling to patients, families, and caregivers.
- Recognize and evaluate situations that call for immediate attention of a physician and institute treatment procedures as necessary.
- Current California NP or PA license.
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification.
- Interest in working with a diverse patient population and commitment to a community health philosophy of prevention and primary care.
- Experience with electronic health records.
- Community Health Center experience.
- Experience as part of a team-based care model.
- EMR Epic experience.
- DEA.
- CAQH PAVE/ORP.
- Bilingual - Spanish or Arabic.
