Physician or Nurse Practitioner/Physician Assistant

We are seeking a highly skilled Physician or Nurse Practitioner/Physician Assistant to join our Homeless Services Department at LifeLong Medical Care. As a key member of our team, you will provide high-quality primary care services to patients experiencing homelessness, working collaboratively with our multidisciplinary team to address the complex health needs of this vulnerable population.
Key Responsibilities- Provide preventive, urgent care, and primary care health services to patients in our Homeless Services Department
- Conduct clinical assessments, order diagnostic evaluations, and develop care plans for patients
- Provide education and counseling to patients, families, and caregivers on health-related topics
- Maintain accurate and up-to-date patient records
- Participate in clinical team meetings and contribute to quality improvement initiatives
- Current California MD, PA, or NP license
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification
- Interest in working with diverse patient populations and commitment to community health philosophy of prevention and primary care
- Experience with electronic health records
- Community Health Center experience
- Experience as part of a team-based care model
- EMR Epic experience
We offer a competitive salary range of $197,000-$218,000/year for MD/DO and $123,000-$146,000/year for APP, pro-rated depending on years since licensed, as well as a sign-on bonus and excellent benefits package.
