Family Nurse Practitioner or Physician Assistant

We are seeking a dedicated and compassionate Family Nurse Practitioner or Physician Assistant to join our team at LifeLong Medical Care. As a primary care provider, you will play a vital role in delivering high-quality, patient-centered care to our underserved communities.
Responsibilities- Provide preventive and primary health services in a Community Health Clinic setting.
- Act as an advocate for clients and consult with other health team members.
- Maintain client records and perform history and physical examinations.
- Order and/or perform laboratory and screening tests, and recommend treatment and medication.
- Provide education, verbal and written, as well as counseling to patients, families, and caregivers.
- Support and participate in triage functions, both by phone and walk-in.
- Current California NP or PA license.
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification required.
- Interest in working with a diverse patient population and commitment to a community health philosophy of prevention and primary care.
- Experience with electronic health records.
LifeLong Medical Care offers a competitive salary range of $123,000-$144,000/year, pro-rated depending on years since licensed, as well as a sign-on bonus and excellent benefits, including medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs.
We are a qualified site for federal and state loan repayment programs (HPSA score is 21). Acceptance rates may vary year to year based on funding levels and the number of applicants.
