About the Role

We are seeking a skilled Physician Assistant or Nurse Practitioner to join our team as a Post-Acute Rehabilitation Specialist. This role will involve providing specialized Physical Medicine and Rehabilitation (PM&R) services to patients in skilled nursing facilities, working closely with the interdisciplinary care team to optimize rehabilitative outcomes and promote patient independence.

Key Responsibilities
  • Provide PM&R services to patients in skilled nursing facilities, including assessing and managing barriers to therapy such as joint or neuropathic pain, spasticity, neurogenic bowel, and neurogenic bladder.
  • Collaborate with PT, OT, and SLP teams to improve patient function and quality of life.
  • Represent Comprehensive Rehab Consultants in the facility through strong clinical leadership and collaboration with the interdisciplinary care team.
Qualifications
  • Nurse Practitioner or Physician Assistant license.
  • California licensure.
  • Ability to work in the U.S.
  • Experience in post-acute space preferred.
