We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Administer skilled nursing care to clients requiring professional services in their homes
- Teach and supervise family members and other members of the nursing team
- Act as primary liaison between physician, patient/family, and home health team
- Provide direct patient care services and supportive care to patients and their families
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Current drivers license and automobile insurance
- Current CPR certification
