Senior Community Care Nurse Practitioner

Optum is revolutionizing the way care is delivered, and we're seeking a skilled Nurse Practitioner to join our Senior Community Care team. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our most vulnerable populations.
Key Responsibilities:- Conduct comprehensive assessments to identify patients' needs and develop personalized care plans.
- Manage patients through acute medical situations, exacerbations of chronic conditions, and short-term sub-acute rehabilitation episodes.
- Ensure accurate and complete documentation, including ICD-10 conditions, to support high-quality care.
- Collaborate with the interdisciplinary care team to provide seamless care coordination.
- Provide advanced illness and advanced care planning conversations to support patients and their families.
- Counsel and educate patients and caregivers on disease management and self-care strategies.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted license in the state where you reside, or ability to obtain by start date.
- Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- 1+ years of clinical experience in practice, with a focus on long-term care, skilled nursing, or assisted living.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
- Access to reliable transportation and availability to work 24 hours per month, with flexibility to work off-hours, weekends, and holidays.
- Clinical experience in long-term care, skilled nursing, or assisted living.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
