Job Summary

We are seeking a highly motivated and experienced Registered Nurse to join our team as a Home Health RN Case Manager. In this role, you will be responsible for providing skilled nursing care to patients in their homes, while also coordinating with healthcare providers and family members to ensure seamless care delivery.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes, as prescribed by physicians.
  • Conduct thorough assessments of patients' needs and develop individualized care plans to meet those needs.
  • Collaborate with healthcare providers and family members to ensure effective communication and coordination of care.
  • Provide education and support to patients and their families on disease management, medication adherence, and other health-related topics.
  • Manage and maintain accurate and up-to-date patient records, including progress notes and care plans.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in home health, hospice, or other relevant settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
