Job Summary

We are seeking a highly skilled Master Counselor to join our team at Bon Secours Community Hospital. The successful candidate will be responsible for coordinating treatment for patients on assigned caseloads, conducting psychosocial evaluations, and participating in formulating patient treatment plans.

Key Responsibilities
  • Develop and implement timely treatment plans and screening evaluations that meet the developmental needs of each patient.
  • Perform comprehensive psychosocial assessments, including clinical impressions and plans for intervention.
  • Utilize the OASAS Level of Care for Alcohol and Drug Treatment Referral (LOCADTR) tool to determine the appropriate level of care and treatment.
  • Provide individual, couple, family, and group clinical treatment as required.
  • Work with patients and families to formulate and review treatment plans and goals.
  • Participate in effective management of emergencies and adjust established work plans as needed.
  • Use individualized treatment approaches to address the needs presented by patients and families.
  • Establish appropriate discharge plans for patients based on the appropriate level of care.
  • Make referrals to adult protective services or child protective services as necessary.
  • Provide education on substance abuse disorders to patients and family members.
  • Seek supervision when required for clinical work and recognize transference issues.
  • Educate other hospital staff, staff at other agencies, patients, and community members about the range of behavioral health services available at Bon Secours Community Hospital and within the community.
  • Protect patient confidentiality through written, oral, and electronic communication.
Requirements
  • Minimum of 1 year experience in chemical dependency preferred.
  • Master's degree in a Human Services field required.
  • NYS Licensure in Social Work or Mental Health Counseling (LMHC, LMSW, LCSW, or Limited Permit) required.
