The Counselor III provides comprehensive treatment and support to patients recovering from addictions, including alcoholism, drug addiction, eating disorders, and other behavioral problems. This role involves assisting patients in modifying problem behaviors and providing counseling services to families of clients.
Roles and Responsibilities:- Participate in patient treatment planning and continuum of care.
- Conduct group and/or family sessions, under supervision, as scheduled and document as required.
- Co-facilitate assigned group or family sessions and assist with modifying problem behaviors.
- Responsible for initial assessments, as well as follow-up assessments for patients.
- Prepare individual treatment plans for each assigned patient.
- Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
- Act as a liaison between referral sources and patients.
- Provide crisis intervention to patients, as needed.
- Plan for aftercare for assigned patients.
- Act as a liaison with insurance carriers, as requested.
- Provide case management duties for patients, ensuring individualized quality care.
- Evaluate the need of the patient and determine if referrals to other programs or facilities are needed.
- Provide patient and family education relative to substance abuse/psychiatric problems.
- Conduct educational workshops both within the Hospital and in the community as required.
- Serve as a liaison with self-help groups and EAP professionals.
- Conduct ongoing assessments of chemical dependence services required by the Hospital treatment programs.
- Assist in the training of new clinical staff and interns, providing direction and clinical guidance.
- Bachelor's degree in social or health services field, with 4+ years of experience in the counseling field.
- OR Bachelor's degree in other field with 6+ years of experience in the counseling field.
- OR Master's degree in social or health services field, with 2+ years of professional work experience.
- Degree must be from an accredited college or university.
- Licensure or certification for counseling may be required by the state in which the facility operates.
- CPR and de-escalation certification required (training available upon hire and offered by facility).
- First aid may be required based on state or facility.
