PROGRAM SUPPORT SPECIALIST (REINTEGRATION)
POSITION SUMMARY
The primary focus of this role is to empower individuals with criminal histories to achieve self-sufficiency through innovative services that foster transformation and improve lives.
The Reintegration Specialist will guide individuals in accessing community resources and pursuing career development and vocational training as needed.
This role serves as a crucial support system for program participants, ultimately aiming to secure meaningful employment.KEY RESPONSIBILITIES
- Collaborate with an interdisciplinary team to ensure that essential services and support are available to assist individuals in setting and achieving their social, vocational, and life skills objectives for successful community reintegration. This task is performed daily, approximately 20% of the time.
- Establish knowledge of and connections with community service providers, including collaborating agencies, probation, parole, court support services, and various local organizations.
Build and maintain relationships with family members, residential providers, funders, and other relevant contacts to ensure that necessary resources are accessible to participants.
This task is performed daily, approximately 20% of the time. - Design and implement educational and training initiatives tailored to participants' needs, including pre and post-release services such as life skills training, financial management, eligibility for assistance programs, socialization, parenting skills, career development, and personal safety. This task is performed weekly, approximately 10% of the time.
- Oversee the intake process for new referrals; conduct needs assessments and interest inventories to create an Individual Service Plan; develop a roadmap to assist each participant in their journey towards self-sufficiency. This task is performed weekly, approximately 10% of the time.
- Provide information and referrals for services not offered by the agency, while maintaining professional and ethical boundaries with clients and community stakeholders at all times. This task is performed weekly, approximately 10% of the time.
- Offer crisis intervention and support in developing responsible behaviors to facilitate successful community reintegration; provide ongoing assistance as needed to ensure active engagement in services outlined in the Individual Service Plan. This task is performed as required, approximately 10% of the time.
- Facilitate discharge planning and follow-up for transitions to other agencies and community programs, including tracking placement and retention. This task is performed weekly, approximately 10% of the time.
- Adhere to client privacy rights and HIPAA regulations; ensure compliance with funding and accreditation standards. This task is performed weekly, approximately 10% of the time.
- Perform any other related duties as assigned.
QUALIFICATIONS
EDUCATION AND EXPERIENCE
Comprehensive knowledge in areas such as accounting, marketing, business administration, and finance.
Equivalent to a four-year college degree, along with 3 years of relevant experience and/or training, or a comparable combination of education and experience.
COMMUNICATION SKILLS
Ability to read, analyze, and comprehend common scientific and technical literature, financial documents, and legal materials; capability to respond to complex inquiries or complaints from clients, regulatory bodies, or members of the business community.

While performing the responsibilities of this role, the employee is continuously required to communicate verbally or audibly; regularly required to stand, walk, sit, use hands for various tasks, reach with hands and arms, and occasionally required to climb, stoop, kneel, crouch, or crawl.
The employee must occasionally lift and/or move up to 25 pounds; frequently lift and/or move up to 10 pounds. Specific vision abilities required for this role include close vision and distance vision.ADDITIONAL INFORMATION
A minimum of 3 years of experience in public service or social services is required. A Bachelor's degree is preferred.
A willingness to learn and respect the needs of individuals with mental health challenges or disabilities is essential.
Knowledge of the criminal justice system and experience working with individuals transitioning from incarceration is highly valued.
Ability to access correctional facilities as needed throughout the designated region is required.
Willingness to travel throughout the designated region is necessary.
