Nurse Care Specialist

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Nurse Care Specialist in our Community Living program. As a key member of our healthcare team, you will provide high-quality nursing care to individuals with intellectual, developmental, or cognitive disabilities.
Responsibilities- Provide nursing care to individuals as prescribed by physicians and in accordance with nursing standards and regulations.
- Conduct comprehensive nursing assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless care coordination.
- Administer medications and treatments as ordered by physicians, and monitor for adverse effects.
- Document individual health status and progress in a timely and accurate manner.
- Participate in health, safety, and infection control committees to ensure a safe and healthy environment.
- Provide in-service training to staff on health services, medications, and side effects.
- Completed post-secondary education coursework to facilitate licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State.
- Ability to read, write, and follow oral and written directions at a level necessary to accomplish the job.
- Ability to relate positively and favorably to individuals and families, and work cooperatively with other associates.

Our community living services are provided in 27 states with several thousand programs and a comprehensive range of high-quality services.
Salary and BenefitsWe offer a competitive salary range of $24.00 to $26.00 per hour, depending on experience. Additionally, we provide a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
