Job Title: RN Home Health Care Specialist

We are seeking a skilled RN Home Health Care Specialist to join our team in the Kansas City, MO area. As a Home Health RN, you will be responsible for providing high-quality nursing care to patients in their homes, working closely with other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities:

  • Provide skilled nursing visits and assessments to patients in their homes
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Monitor and report patient progress to healthcare teams and families
  • Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

  • Active Registered Nurse license in the state of MO or Compact
  • Home Health experience required
  • Strong organizational and self-management skills
  • Excellent communication skills
