Job Title: Licensed Practical NurseJob Summary:

IntelyCare is seeking a Licensed Practical Nurse (LPN) to join our team in Branford, CT. As an LPN with IntelyCare, you will have the opportunity to work with a supportive team, enjoy flexible scheduling, and receive competitive pay and benefits.

Job Responsibilities:
  • Deliver post-acute care to residents
  • Monitor physical and mental wellbeing of residents and track vital signs
  • Perform intakes of residents and document health concerns and medical histories
  • Review medical records and charts of residents
  • Administer and verify medications
  • Maintain appropriate levels of medical supplies
  • Contribute to a safe and comfortable environment for residents
  • Assist with IV set up, vaccinations, and phlebotomies
  • Follow and adhere to care plans for residents
  • Help medical staff with orientation and training
Qualifications and Skills:
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
