About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with IntelyCare, you will have the opportunity to work in a variety of post-acute care settings, including skilled nursing facilities, assisted living communities, and home health care.

Responsibilities
  • Provide high-quality patient care, including administering medications, monitoring vital signs, and documenting patient information
  • Collaborate with healthcare teams to develop and implement individualized care plans
  • Conduct thorough assessments of patients' physical and mental health needs
  • Develop and implement effective treatment plans to address patients' health concerns
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved LPN educational program
  • High school diploma or equivalent
  • At least 1 year of experience as an LPN or in a related role
