Job Description:

We are seeking a licensed practical nurse to join our team in Branford, CT. As an LPN with IntelyCare, you will have the opportunity to work flexible shifts and earn competitive pay. Responsibilities:
  • Deliver post-acute care to patients
  • Monitor patients' physical and mental well-being
  • Perform intakes and document patients' health concerns and medical histories
  • Review patients' medical records and charts
  • Administer medications and verify their accuracy
  • Maintain appropriate levels of medical supplies
  • Contribute to a safe and comfortable environment for patients
  • Assist with IV set up, vaccinations, and phlebotomies
  • Be accountable for following and adhering to care plans for patients
  • Help medical staff with orientation and training
Qualifications and Skills:
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
