About the Role

We are seeking a skilled and experienced LPN to join our team of healthcare professionals. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities, providing high-quality patient care and making a real difference in the lives of those you serve.

Responsibilities
  • Provide exceptional patient care, including tracking wellbeing, monitoring vital signs, and administering medications
  • Conduct intakes, document health concerns, and review medical records
  • Collaborate with healthcare teams to ensure safe and comfortable facilities
  • Participate in IV set up, immunizations, and phlebotomy procedures
  • Assist with the orientation and training of medical staff
Requirements
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved LPN educational program
  • High school diploma (or GED/equivalent)
  • At least 1 year of experience (LPN or related role)
