Job Summary

We are seeking a skilled Clinical Team Lead to join our team at Southern Home Care Services, Inc. As a Clinical Team Lead, you will be responsible for overseeing the day-to-day operations of our nursing services department, ensuring that our team members are meeting performance goals and providing high-quality care to our patients.

Key Responsibilities

  • Lead and manage a team of nursing professionals to ensure successful implementation of clinical programs and services
  • Develop and maintain relationships with healthcare providers and partners to ensure seamless care coordination
  • Monitor and analyze patient outcomes and make data-driven decisions to improve care quality and efficiency
  • Collaborate with cross-functional teams to develop and implement clinical policies and procedures
  • Provide coaching and training to team members to enhance their clinical skills and knowledge

Requirements

  • Bachelor's degree in Nursing or related field
  • Minimum of 3 years of experience in a clinical leadership role
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Strong analytical and problem-solving skills
