Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team at ComForCare. As a Home Care Specialist, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Key Responsibilities
  • Conduct comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develop and implement individualized plans of care that incorporate analysis of data and current scientific findings.
  • Provide interventions with a focus on achieving realistic client outcomes.
  • Manage and administer medications, and provide wound care as needed.
  • Report changes and updates to the plan of care to reflect progress towards goals and outcomes.
  • Initiate referrals as needed to other health team members.
Requirements
  • Current license as a Registered Nurse (RN).
  • Reliable transportation and willingness to commute to clients' homes.
  • Caring, empathetic, friendly, and dynamic personality.
