About the Role

VNA Health Group is seeking a skilled Speech Language Pathologist to join our team of healthcare professionals. As a key member of our home health team, you will be responsible for providing high-quality speech and language therapy services to patients in their homes or community settings.

Responsibilities
  • Assessment and Evaluation: Conduct thorough assessments and evaluations of patients to identify their speech and language needs and develop personalized plans of action.
  • Therapy Services: Provide evidence-based speech and language therapy services to patients in their homes or community settings, including individual and group sessions.
  • Documentation and Communication: Maintain accurate and detailed records of patient progress and communicate effectively with patients, families, and healthcare teams.
  • Collaboration and Teamwork: Work collaboratively with other healthcare professionals to ensure seamless care coordination and patient outcomes.
Requirements
  • Education and Certification: Master's degree in Speech-Language Pathology and CCC-SLP certification.
  • Experience: 2-3 years of prior clinical experience and community-based experience preferred.
  • Skills: Strong communication and interpersonal skills, valid NJ driver's license, and ability to work independently and as part of a team.
