About the Job:

We are seeking a highly skilled Referral Partnerships Coordinator to join our team at Community Servings. The successful candidate will be responsible for managing and growing relationships with referral partners, including health centers, physician practices, and other organizations. This role requires a strong understanding of the healthcare industry and excellent communication skills.

**Key Responsibilities:**
  • Manage and grow relationships between Community Servings and referral partners through regular meetings, communications initiatives, and other outreach activities.
  • Work closely with the Program team to ensure coordinated handoff of referrals.
  • Collaborate with the Communications team to develop outreach collateral and processes for assisting referral partners and clients to easily access our services.

Requirements:
  • 2-3 years of experience in outreach, healthcare sales, or nonprofit work.
  • Demonstrated sales and/or project management experience.
  • Ability to work independently and as part of a team.
