We are seeking an experienced Registered Nurse Home Health to join our team at AuthoraCare Collective. This is a full-time position that requires excellent patient assessment and clinical nursing skills, critical thinking, problem-solving, delegation, effective interpersonal and communication skills.

About the Role:
As a Registered Nurse Home Health, you will be responsible for managing the overall Plan of Care for a caseload of Integrated health patients in collaboration with other members of the interdisciplinary team (IDT). You will assess and identify patient/caregiver problems, establish nursing goals, and interventions. You will work closely with physicians to establish medical Plans of Care for patients and establish and maintain therapeutic relationships with patients, including providing disease education, medication management, and health recovery.

Key Responsibilities:
• Manage the overall Plan of Care for a caseload of Integrated health patients
• Assess and identify patient/caregiver problems, establish nursing goals, and interventions
• Work closely with physicians to establish medical Plans of Care for patients
• Establish and maintain therapeutic relationships with patients
• Provide disease education, medication management, and health recovery

Requirements:
• Graduate of an accredited School of Nursing
• Currently licensed as an RN in North Carolina and in good standing with NCBON
• One year of experience in an acute, hospice, home health setting, or long-term care setting; two years preferred
• Bachelor's degree in nursing preferred
• CPR Certification
• Valid state-issued driver's license required; must carry automobile liability insurance at limits required by agency; must have own transportation

Benefits:
• Competitive salary: $65,000 - $85,000 per annum
