Clinical Case Manager

Lifepoint Health is seeking a skilled Clinical Case Manager to join our interdisciplinary team in North Smithfield, Rhode Island.
About the RoleAs a Clinical Case Manager, you will play a vital role in ensuring that patients receive comprehensive and coordinated care throughout their rehabilitation journey. This includes completing departmental orientation, initial and annual competencies, consulting with other departmental staff to collaborate in patient care delivery, identifying barriers to care and or discharge, and developing solutions/resolutions.
You will be responsible for scheduling family conferences and/or communicating with caregivers following each team conference and more often as needed to keep patients and designated caregivers informed of progress and provides appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge. Additionally, you will coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans to achieve identified patient goals and outcomes.
We are seeking an individual with a strong background in case management, preferably with certification in Case Management or Rehabilitation Nursing. Your effective oral and written communication skills in English are essential for this role, with additional languages being a plus. Basic computer skills in Excel, Word, Outlook, PowerPoint, etc., are also required.

Job Requirements- Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist
- Certification in Case Management or Rehabilitation Nursing preferred
- Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred
- Effective oral and written communication skills in English; additional languages preferred
- Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required
