Position Title: Advanced Practice Practitioner for Navigation Center

Department: Medical Administration

Reports To: Medical Director

FLSA Status: Exempt

Expected Compensation: $5,736.82 bi-weekly

Position Summary:

The Advanced Practice Practitioner is responsible for overseeing clinical operations within the Navigation Center, reporting directly to the Medical Director. This role involves collaborating with supervising physicians under the guidelines of the California Medical Board, ensuring compliance with clinical protocols, and facilitating effective communication between various healthcare providers to address patient needs. The practitioner will provide care through Telemedicine and manage medication refill requests as necessary.

Key Responsibilities:

  1. Ensure that call center personnel adhere to established clinical guidelines and policies to promote efficient patient care. Act as a liaison between physicians and other departments for referrals and walk-ins, playing a crucial role in triaging patient concerns to prevent unnecessary Emergency Room visits.
  2. Conduct triage, diagnosis, treatment, and referrals for patients in an outpatient or telemedicine setting. Prescribe medications and treatment plans as appropriate, consulting with supervising physicians when necessary.
  3. Maintain comprehensive health records for each patient in accordance with the Center's Health Information Management policies, ensuring all documentation is complete and submitted within the required timeframe.
  4. Provide timely medical services in alignment with the clinic schedule and within the scope of practice for Nurse Practitioners.
  5. Review lab results, manage medication refill requests, and assess imaging reports for patients under care, ensuring timely communication.
  6. Deliver exceptional customer service through various communication methods, including telephone consultations and face-to-face interactions.
  7. Provide clinical oversight to support staff involved in relevant activities and procedures.
  8. Adhere to departmental policies and participate in continuous quality improvement initiatives.
  9. Collaborate with other departments to develop necessary policies and procedures in the absence of established guidelines, demonstrating leadership and project management skills.
  10. Engage in health promotion and education programs as required, participating in clinic meetings and other functions as directed.
  11. Perform additional duties as assigned.

Qualifications:

Master's Degree in Nurse Practitioner or Physician Assistant Program from an accredited institution.

Prior experience as a Family Nurse Practitioner or Physician Assistant is highly preferred.

Current unrestricted license to practice as a Nurse Practitioner or Physician Assistant in California.

Annual continuing medical education as mandated by board specialty.

Registered with a current DEA license.

Experience in Telemedicine is highly preferred.

Bilingual proficiency in English and Spanish is highly desirable.

Certification in Basic Life Support (BLS) from the American Heart Association.

Work hours and locations may vary.

Supervisory Responsibilities:

May supervise students under the direction of the Medical Director.

Skills and Abilities:

  • Active California Medical Physician Assistant (PA-C) or Certified California Registered Nurse Practitioner (CRNP) License.
  • Active DEA License.
  • Master's degree in Physician Assistant studies or Nurse Practitioner program from an accredited institution.
  • Minimum of one year of postgraduate experience in a clinical outpatient setting.
  • Preferred: Certification in Psychiatric-Mental Health for Nurse Practitioners and Certificate of Added Qualification (CAQ) for Physician Assistants.
  • Strong motivation to develop clinical skills in psychiatric medicine.
  • Comprehensive understanding of DSM-V diagnoses and criteria.
  • Basic computer and internet proficiency.

Language Skills:

Ability to interpret complex documents and respond effectively to sensitive inquiries. Bilingual ability in English and Spanish is desirable.

Mathematical Skills:

Ability to perform basic mathematical operations and interpret data.

Reasoning Ability:

Strong knowledge of psychiatric and family-centered medicine principles. Ability to apply logical reasoning to a variety of problems.

Certificates, Licenses, Registrations:

Possession of a current unrestricted NP/PA license from the Medical Board of California and a valid DEA license. Certification in Basic Life Support (BLS) is required.

Physical Demands:

Physical requirements include the ability to use hands, stand, walk, and occasionally lift up to 30 pounds. Vision abilities required include close, distance, and peripheral vision.

Work Environment:

Must be flexible with work hours, including potential evenings and weekends, and be willing to travel to various clinic locations as needed.
