About the Role

We are seeking a highly organized and detail-oriented Hospice Team Coordinator to join our team at BrightSpring Health Services. As a key member of our hospice team, you will play a vital role in maintaining scheduling, patient records, and supplies, ensuring seamless care delivery to our patients and their families.

Responsibilities
  • Maintain accurate and up-to-date administrative files and patient records in accordance with Medicare regulations and company policies.
  • Process new patient referral information in a timely and efficient manner.
  • Order and maintain adequate inventory of medical equipment and supplies, ensuring timely delivery and minimizing waste.
  • Provide exceptional customer service by answering incoming calls, coordinating mail, and welcoming visitors in a professional and courteous manner.
  • Complete assigned workflow tasks and reports in our electronic medical record (EMR) system, HCHB, and set up tablets for clinicians as needed.
  • Maintain and update accurate schedules for field staff, ensuring timely and efficient care delivery.
  • Communicate changes related to scheduling, admissions, and discharges to staff, ensuring seamless care transitions.
  • Assist Human Resources in maintaining accurate employee records, ensuring compliance with company policies and procedures.
  • Ensure compliance with all state and federal legal and regulatory requirements, maintaining a high level of professionalism and integrity.
Qualifications
  • 2 years of administrative healthcare experience, with a preference for experience in hospice or home health.
  • Experience working with EMR systems, with HCHB experience being a plus.
  • Ability to deal tactfully with patients, families, and the community, demonstrating empathy and compassion in all interactions.
