About the Role

We are seeking a highly motivated and compassionate Case Manager to join our Street Outreach team. As a Case Manager, you will provide critical support and advocacy services to runaway, homeless, and at-risk youth, helping them navigate the complexities of their lives and achieve stability and self-sufficiency.

Responsibilities
  • Provide case management and advocacy services to youth, including assessment, goal setting, and service coordination
  • Collaborate with internal and external partners to identify and address barriers to youth accessing community resources
  • Support youth in developing self-advocacy skills and building resilience
  • Participate in team meetings and contribute to the development of program strategies and initiatives

Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, or related field
  • 4 years of experience in human services, or a combination of education and experience
  • Experience working with at-risk youth and group facilitation
  • CPR certification and ability to obtain a Class E Driver's License
