Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes to promote their plan of care. This includes assessing, planning, implementing, documenting, and evaluating care to patients and families. The LPN supplements the nursing care needs of patients as provided by the Registered Nurse (RN).

These services are performed in accordance with physicians' orders and in collaboration with Haven Home Health RNs, therapists, and other clinicians.

Key Responsibilities:

  • Recognize and report changes in the patient's condition to the RN supervisor.
  • Complete timely and accurate clinical notes, including addressing patient progress.
  • Assist the RN in carrying out the patient plan of care.

Requirements:

  • Current Missouri LPN license.
  • At least one year of recent LPN experience in acute, long-term, or home care, preferred.
  • Home healthcare experience preferred.
