Job Summary

Haven Home Health & Hospice is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As an LPN with Haven, you will play a vital role in promoting the patient's plan of care, assessing, planning, implementing, documenting, and evaluating care to patients and families.

Responsibilities
  • Recognize and report changes in the patient's condition to the RN supervisor.
  • Complete timely and accurate clinical notes, including addressing patient's progress.
  • Assist the RN in carrying out the patient plan of care.
Qualifications
  • Current Missouri LPN license.
  • At least one (1) year recent LPN experience in acute, long term or home care preferred.
  • Home healthcare experience preferred.
