About the Role

We are seeking a skilled and compassionate Hospice Admissions Nurse to join our team at LHC Group. As a Hospice Admissions Nurse, you will play a critical role in the execution of the hospice admission process, ensuring compliance with federal and state regulations.

Key Responsibilities

  • Advocate for patients and their families to meet their stated needs and provide end-of-life care.
  • Develop trusting relationships with patients and their families as an end-of-life care expert.
  • Maintain confidentiality and safety of patient/client clinical records.
  • Complete assigned admissions and patient care orders in an effective and timely manner.
  • Communicate effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding new admissions.
  • Record relevant information to determine hospice eligibility.
  • Communicate with operational, clinical, and sales leadership regarding status of pending referrals.

Requirements

  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive patient information.
