Recovery Support Coordinator Role

The United Community Center seeks a skilled Recovery Support Coordinator to provide comprehensive recovery support coordination services to clients. The ideal candidate will possess a strong understanding of case management principles and be able to establish contact with clients within 24 hours of referral.

Key Responsibilities:

  • Form a Recovery Support Team with clients and collaborate with formal and informal supports using the Wraparound process.
  • Connect clients with community resources to address needs and goals.
  • Advocate for clients and collaborate with other systems and services.
  • Complete and submit Service Authorization Requests and Justifications.
  • Monitor client needs and consider discharge and referral to other services as necessary.

Requirements:

  • Associate's Degree in Criminal Justice, Social Work, Human Services, or related field.
  • Experience in case management or related community work.
  • Knowledge of community resources and natural supports.
  • Ability to manage multiple job responsibilities and clients simultaneously.
