Job Summary:

The Recovery Navigator is a critical role in the Hub and Spoke Health Home, serving individuals with significant substance use and co-occurring mental health needs. This position requires a strong focus on engagement, advocacy, and collaboration to ensure individuals receive comprehensive and quality care.

Key Responsibilities:
  • Develop and implement individualized comprehensive care plans in collaboration with the individual and their care team.
  • Facilitate monthly recovery monitoring and team meetings to ensure progress and address ongoing needs.
  • Coordinate care with providers and verify services are provided, including transportation and attendance at appointments.
  • Work closely with individuals to develop a team of informal and formal supports based on their preferences.
  • Assist with planning and implementing monthly support groups and educational events.
  • Provide crisis response and develop strategies to meet immediate needs, including relapse prevention planning.
  • Complete satisfaction surveys with individuals in the program and document all interactions and activities in the electronic medical record.
Requirements:
  • High school diploma or equivalent required; Bachelor's degree in Social Work, Psychology, Nursing, or Social Services preferred.
  • Experience serving individuals with substance use and co-occurring needs, with knowledge of the Milwaukee County substance use system and evidence-based practices.
  • Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
Working Conditions:

The Recovery Navigator will work in an office and community environment, with frequent travel to serve individuals in their homes, courts, and other locations. A flexible work schedule is required to meet the needs of staff and individuals served.
