Job Overview

Transitional Services for New York, Inc. is seeking a Medical Coordinator to join our team at one of our residential locations in Queens Village, NY. As a Medical Coordinator, you will play a vital role in ensuring the medical and dental care needs of our residents are met.

Key Responsibilities

  • Coordinate and monitor the medical and dental care for all residents
  • Advocate and intermediary for residents' rights to quality care
  • Work in partnership with residents and program staff to ensure continuity of care
  • Ensure residents receive required routine and emergency medical and dental care
  • Provide assistance with scheduling appointments and escorting individuals to appointments
  • Monitor follow-up to appointments and ensure care is appropriately documented
  • Ensure residents have an adequate supply of medication and medication supplies
  • Complete TSINY #123 forms weekly and facilitate Medication Management Training Groups with residents
  • Maintain contact with treatment providers to ensure integrated care
  • Meet with the interdisciplinary team for assessment and information on residents
  • Visit hospitalized clients as necessary and provide crisis management when required

Requirements

  • Bachelor's degree or Associate's degree plus one year's experience or High School diploma plus two years' experience in a mental health setting
  • Detail-oriented with the ability to work independently and take initiative
  • Effective oral and written communication skills with the ability to interact with all levels of employees and external contacts
  • Ability to handle confidential and sensitive information
  • Proficient with basic office equipment and able to work effectively using program management tools
