LPN Hospice Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at MJHS Hospice & Palliative Care. As a Hospice LPN, you will play a vital role in providing high-quality care to patients and their families in their homes or facilities.
About the RoleUnder the direction of an RN Case Manager, you will visit patients in their homes or facilities, providing direct care and documenting each visit using our electronic medical record system. You will also report any changes in the patient's condition to the RN Case Manager.
Responsibilities- Provide direct care to patients in their homes or facilities
- Document each visit using our electronic medical record system
- Report any changes in the patient's condition to the RN Case Manager
To be successful in this role, you will need:
- A graduate degree from an accredited Licensed Practical Nurse program
- A minimum of one year of experience as an LPN in med/surg nursing and one year of hospice or oncology nursing experience
- Good PC skills and a valid driver's license
