Overview

Position Summary

This role is responsible for overseeing the clinical services delivered in alignment with established standards of patient care as defined by regulatory bodies, accreditation organizations, and internal policies. The position ensures the effective management of resources to meet patient requirements while achieving the operational, financial, and quality objectives of the branch. Collaboration with leadership is essential to enhance care and services across the organization, ensuring efficiency and effectiveness in operations.

Key Responsibilities

  • Provides administrative and clinical support, serving as a vital link between physician practices, hospital case management, and other referral sources to address patient and staff needs.
  • Analyzes data to prioritize complex situations, enabling appropriate actions based on the urgency of patient health, safety, and the effectiveness of service delivery.
  • Identifies obstacles to resolving issues and collaborates with relevant resources or leadership to ensure timely and effective solutions.
  • Reviews and aids in the creation of patient-specific clinical care plans, ensuring clear communication among all parties involved to support positive patient outcomes and satisfaction.
  • Assists clinical staff in executing the established care plan, coordinating service delivery, and implementing strategies that promote health and safety.
  • Monitors progress towards achieving outcomes through documentation review, interdisciplinary meetings, and case conferences.
  • Ensures compliance with billing processes by staying informed about payer requirements and regulations, facilitating timely completion of workflows, and identifying opportunities for reimbursement.
  • Delegates care responsibilities to appropriate healthcare personnel in accordance with legal and policy guidelines.
  • Engages with patients, families, and other stakeholders in the clinical practice process.
  • Utilizes resources effectively to plan and deliver clinical services that are safe, efficient, and financially sound.
  • Participates in committees, shared governance, or other decision-making activities within the facility.
Physical Requirements
The primary work environment is an office setting, which requires prolonged periods of sitting and computer use. The role may involve walking, standing, lifting, reaching, bending, pushing, and pulling. Candidates must be able to lift and support up to 35 pounds when handling patients, medical equipment, and supplies. Proficiency in English is required, along with intact sensory functions and fine motor skills. Critical thinking and the ability to respond swiftly to changes in patient conditions are essential.

Qualifications

Candidates must be graduates of an accredited nursing program, with a BSN preferred. BLS certification is required in accordance with policy guidelines. A minimum of one year of experience in a related field is necessary. A current RN license or temporary license as a Registered Nurse in the state of practice is required. Maintenance of a valid state driver's license and auto insurance is mandatory. Additional education, training, certifications, or experience may be required as determined by the nursing leadership.
