Job Summary: We are seeking a skilled Behavioral Intake Specialist to join our team at Humboldt Park Health. As a key member of our admissions team, you will be responsible for managing inquiries, conducting clinical screenings, and coordinating pre-admission activities.

Key Responsibilities:

  1. Manage all inquiries to the department related to admission referrals from internal and external customers and document on the designated form.
  2. Conduct an initial clinical screening for admission requests and collect preliminary demographics and verify insurance benefits.
  3. Provide assistance to the HPH, ER physician in assessment of patients to determine eligibility for admission based on established admission criteria.
  4. Provide appropriate outpatient referrals for ER patients when directed by the ER attending.
  5. Coordinate all pre-admission activities with the BMU nursing unit for patients being considered for admission, including validation of required legal paperwork, gathering of other pertinent information.
  6. Coordinate all activities for patients accepted for admission, including completion of admission entry paperwork with the patient.
  7. Identify patients' presenting problems and complete the preliminary treatment plan.
  8. Maintain various logs of activities and participate in department/intake performance improvement initiatives.

Requirements: Some experience in psychiatric triage or patient assessment is essential. Must have experience with the chronically mentally ill population. A Bachelors Degree is required.
