LPN Home Health Per Diem Nurse

CenterWell Home Health is seeking a skilled and compassionate LPN to join our team of healthcare professionals. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them.
- Document patient progress, maintain accurate records, and communicate with the healthcare team as needed.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- HCHB (Home Care Home Base) EMR experience a plus.
- One year experience as an LPN in Skilled Home Health Visits preferred.
