We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Concierge Home Care. As a Home Health Nurse, you will provide high-quality patient care and support to individuals in the comfort of their own homes.
Job Summary:The Home Health Registered Nurse will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and excellent patient outcomes.
Responsibilities:- Conduct thorough patient assessments to identify needs and develop personalized care plans
- Provide skilled nursing care, including wound care, medication management, and pain management
- Collaborate with patients, families, and healthcare providers to ensure comprehensive care
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and training to stay current with best practices and industry developments
- Graduate of an accredited school of nursing
- Current RN licensure in the state of Florida
- Strong assessment and care planning skills
- Ability to work independently and as part of a team
