Job Summary:

The Licensed Practical Nurse is responsible for delivering skilled nursing care under the direct supervision of a Registered Nurse. You will perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Your primary goal is to provide exceptional patient care and support, ensuring the best possible outcomes for our patients.

Key Responsibilities:
  • Carry out the plan of care under the supervision of the Registered Nurse.
  • Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
  • Assist with the preparation, implementation, and continuing evaluation of the client care plan.
  • Recognize and understand the effects of social and economic problems upon clients and provide for the emotional and physical comfort and safety of clients.
  • Help teach the patient the appropriate self-care techniques.
  • Foster cooperative effort among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
  • Prepare clinical and progress notes (not to include admission). Includes interim orders written with measurable goals and time frames and the integration of these orders into the patient's plan of care under the supervision of a Registered Nurse.
  • Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, including:
  • The general physical and mental condition of clients and signs and symptoms which may be indicative of untoward changes.
  • Facilitate relationships between the clients and staff personnel and between clients and their families and visitors.
  • Observe, record, and report to the appropriate person the general physical and mental condition of clients and signs and symptoms which may be indicative of changes in condition.
  • Assist with the rehabilitation of clients according to the medical care plan by encouraging the interests and special aptitudes of the client and encouraging clients to help themselves using their own capabilities.
  • Further assist in rehabilitation by reporting any abnormality in the client's range of motion, body mechanics, or body alignment.
  • Actively participate in nursing organizations and promote and participate in in-service programs.
  • Awareness of the Agency's QAPI program. Active participation as required per policies and procedures related to the Agency's QAPI program.
  • Management and follow-up of customer complaints, following Agency's complaint policy.
  • Acute awareness of patient rights and the Agency's responsibility in honoring patient's rights. Awareness of patient rights description in Agency's service agreement and Agency's admission packet.
  • Awareness of appropriate reasons for a patient discharge and Agency's policy on appropriate reasons for discharge.
  • Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
  • Awareness and ability to design and identify patient-centered and individualized goals with realistic measurable timeframes under the supervision of a Registered Nurse.
  • Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency's policy and procedures. Awareness and participation in the Agency's infection control program.
  • Nursing bag technique policy acknowledgement and demonstrated competence.
  • Utilization of CRT, calculator, multi-line telephone, copy machine, basic medical equipment.
Requirements:
  • IV certification.
  • The ability to communicate well, both verbally and in writing, is required.
  • The ability to access client's homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • Physical activities will include walking, sitting, stooping, and standing, and minimal to maximum lifting of clients and the turning of clients.
  • Graduation from an accredited School of Practical Nursing.
  • Must be licensed in the state of Licensed Practical Nurse.
  • One or more years in an acute/sub-acute care facility. Home health experience preferred.
