LPN Home Health Per Diem Nurse

We are seeking a skilled and compassionate LPN to join our team at CenterWell Home Health. As a Home Health LPN, you will provide high-quality nursing care to patients in their homes, promoting independence and improving their overall health and well-being.
Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans, utilizing a holistic approach to address patients' physical, emotional, and social needs.
- Conduct thorough assessments, identify patient needs, and develop effective interventions to achieve desired outcomes.
- Collaborate with the healthcare team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers, providing education and support to promote self-care and independence.
- Document patient progress, outcomes, and any changes in their condition, ensuring accurate and timely reporting.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- HCHB (Home Care Home Base) EMR experience a plus.
- One year experience as an LPN in Skilled Home Health Visits preferred.
