We are seeking a skilled and compassionate Home Health LPN to join our team at HCA Florida Healthcare at Home. As a Home Health LPN, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.
Responsibilities- Administer skilled nursing care to patients in their homes, under the direction of a Registered Nurse and/or Director of Professional Services.
- Conduct ongoing assessments of patients' physical, psychosocial, and environmental needs, and document data in patient medical records.
- Develop and implement individualized plans of care, incorporating short and long-term goals, and provide education to patients and caregivers as needed.
- Communicate effectively with patients, caregivers, and healthcare team members to ensure seamless care coordination.
- Graduate of an accredited LPN/LVN program.
- Minimum of one year experience as an LPN/LVN in a clinical care setting.
- Home care experience preferred.
- Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services.
- Current BCLS prior to delivering patient care.
- Reliable transportation and proof of valid automobile liability insurance.
