Job Title: RN - Case Management

Prime HealthCare Staffing is seeking an experienced RN - Case Management to join our team at Kindred Hospital Seattle - First Hill - SCION HEALTH. As a RN - Case Management, you will play a vital role in ensuring the best possible outcomes for our patients.

Responsibilities:
  • Develop and implement effective case management plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments and identify opportunities for improvement in patient care.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
Requirements:
  • Current Washington state nursing license.
  • At least 2 years of recent case management experience as a RN - Case Management.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
