Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at UofL Health. The successful candidate will provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes for patients.

Key Responsibilities
  • Provide comprehensive pharmaceutical care to patients, including medication order evaluation, verification, and documentation.
  • Collaborate with multidisciplinary teams to deliver high-quality patient care and improve therapeutic outcomes.
  • Develop and implement clinical pharmacy services, including formulary management, medication therapy management, and patient counseling.
  • Participate in quality improvement initiatives and contribute to the development of new programs and services.
  • Supervise and precept pharmacy students, residents, and technicians to ensure the delivery of high-quality patient care.
  • Stay up-to-date with contemporary trends in pharmacy practice and the profession through ongoing education and professional development.
Requirements
  • Graduation from an accredited College of Pharmacy.
  • Current licensure as a pharmacist in the Commonwealth of Kentucky.
  • Board certification by the Board of Pharmacy Specialties within 2 years from date of hire.
  • Current Basic Life Support and Advanced Cardiac Life Support certification.
  • Post-Graduate Year 1 (PGY-1) Pharmacy Residency and Post-Graduate Year 2 (PGY-2) Solid Organ Transplant Residency preferred.
