- Education and Experience: A Bachelor's Degree in Pharmacy and a minimum of one year of experience as a pharmacist, or a Doctorate Degree in Pharmacy with a minimum of 5 years of experience.
- Professional Development: Completion of an accredited Post Graduate Year-1 (PGY-1) residency, or Board Certification in Pharmacotherapy.
