About the Role

We are seeking a highly skilled and compassionate Licensed Mental Health Therapist to join our team at Community Healthcare Partners. As a key member of our team, you will have the opportunity to make a positive impact on the lives of our patients and their families.

Key Responsibilities
  • Assess and evaluate patients to formulate a personalized plan of care in coordination with the patient's caregiver, healthcare team, and community resources/referrals.
  • Work in conjunction with our other Mental Health Therapist and Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide comprehensive care to our patients.
  • Help patients understand, identify, and set therapeutic goals for concerns such as trauma, mental/personality/behavioral disorder, and family dysfunction.
  • Communicate effectively with patients, caregivers, and healthcare teams to ensure seamless care coordination.
Requirements
  • Active full license or provisional license as a Licensed Mental Health Therapist (LIMHP) in the state of Nebraska.
  • Two (2) years of experience as a Licensed Mental Health Therapist providing direct psychotherapy services to individuals and families in an outpatient setting preferred.
  • Ability to confidently work with youth, adult, and geriatric populations.
  • Solid computer and basic technology skills in order to complete clinical documentation with 48 hours of service and incident reports within same day.
  • Licensed and insured driver with an automobile that is in good working order and ability to drive various weather conditions to ensure reliability.
