Reports To: Children's Coordinator

Position Overview: The Family Support Advocate offers specialized assistance to caregivers of children experiencing Serious Emotional Disturbance (SED), Substance Use, or co-occurring conditions. This role is designed to facilitate the stabilization of the child and improve the overall quality of life for the family. The Advocate must possess personal experience in raising a child with SED, Substance Use, or co-occurring conditions and is required to complete the necessary certification and training to become a certified Family Support Advocate.

Essential Responsibilities:

  1. Complete training and certification promptly as directed by the supervisor.
  2. Devote 62.5% of work hours to providing direct support services.
  3. Establish and maintain a professional relationship with Families Together, utilizing them as a resource.
  4. Conduct face-to-face meetings with caregivers to provide interventions aimed at achieving identified goals.
  5. Ensure timely and accurate documentation of all services, including mileage and electronic timesheets.
  6. Maintain precise and medically necessary records of service delivery through individualized progress notes, completed in collaboration with caregivers.
  7. Submit progress notes on the same day of service or by 9:00 AM the following business day, with Friday notes due by the end of the day.
  8. Assist caregivers during their child's hospitalization, focusing on the transition back home and ensuring follow-up care within three days post-discharge.
  9. Support caregivers in problem-solving, accessing resources, completing referrals, and coordinating ancillary services.
  10. Act as a liaison between service providers and caregivers to facilitate service coordination and understanding of treatment needs.
  11. Engage in the treatment planning process with families to ensure caregivers are supported and involved in goal development.
  12. Help caregivers access necessary medical, social, educational, and employment services, including arranging transportation.
  13. Utilize evidence-based strategies to enhance caregiver knowledge and advocate for the family.
  14. Monitor the status of the youth and provide appropriate support to caregivers based on the youth's needs.
  15. Communicate effectively with Case Managers and other service providers to maintain a collaborative approach.
  16. Assist caregivers in crisis situations and develop crisis plans in collaboration with the assigned Case Manager.
  17. Provide transitional care for caregivers following school suspensions or expulsions, evaluating behaviors and current services.

  1. Maintain a satisfactory attendance record, including participation in meetings and training.
  2. Exhibit a suitable level of technical knowledge for the role.
  3. Produce sufficient work volume to meet job expectations.
  4. Collaborate effectively with team members and maintain respectful interactions.
  5. Demonstrate professional communication skills with peers and supervisors.
  6. Adapt to new assignments and changes in procedures as needed.
  7. Complete tasks independently and meet deadlines without reminders.
  8. Perform duties with a high degree of accuracy.
  9. Achieve goals set during performance reviews.

Position Requirements: Candidates must have personal experience in raising a youth with SED, Substance Use, or co-occurring conditions. Proficiency in computer skills is required. Preferred qualifications include knowledge of youth mental health, school resources, community services, and vocational options; strong verbal and written communication skills; and the ability to build rapport with youth, families, and staff. A valid driver's license and access to a personal vehicle are necessary.
